CQB City Waiver

A completed liability waiver is required for each player attending any CQB City event. Each participant must fill out a waiver PRIOR to arriving at the facility. NOTE: Operators only need to submit one Liability waiver per player per year! Operators 13 to 17 years of age MUST have a PARENT or LEGAL GUARDIAN complete the Liability waiver Only Pre Paid Operators will be guaranteed entrance.
